His age of 2 months and 4 days, his date and place of birth and death, his parents' names: Stanislaw & Mary (Matuszak) Luczyk, and the name of the cemetery in which he was buried are given in his Indiana death certificate for which his father was the informant. The undertaker was Bro. Francis Xavier of Notre Dame.
