SOUTH BEND - David W. Levendoski, 59, died on Wednesday at his residence. His wife, Wanda (Wentworth) survives, along with his children, Deanne Levendoski, Bethany Levendoski, Amber (Caleb) Henderson, and Christopher (Sarah) Dionne. Visitation will be from 2-8 pm on Monday at ST. JOSEPH FUNERAL HOME, 824 S. Mayflower Rd., where a Rosary will be prayed at 4:30 pm. A Mass of Christian Burial will be celebrated at 10 am Tuesday at Holy Family Catholic Church. Burial at St. Joseph Cemetery. www.sjfh.net

SOUTH BEND - David W. Levendoski, 59, passed away at 6:10 pm on Wednesday, November 23, 2011 in his residence, with his family by his side. David was born on May 29, 1952 in Gary, IN to the late Robert W. and Mary Jane (Surowiec) Levendoski. On August 18, 2007 he married Wanda (Wentworth); she survives along with his children, Deanne Levendoski of South Bend, IN, Bethany Levendoski of Chicago, IL, Amber (Caleb) Henderson of Kailua, HI, and Christopher (Sarah) Dionne of St. Joseph, MO; three grandchildren, Kingston William Levendoski of South Bend, IN, Lucas Aiden Dionne and Lydia Kate Dionne both of St. Joseph, MO; two sisters, Jane (Paul) Bock Of Naperville, Il and Joan (Dave) Potts of Georgetown, KY; and by his three brothers, Robert (Korrine) Levendoski of Mansfield, OH, Richard Levendoski of Columbus, OH and Joseph (Anne) Levendoski of New Berlin, WI. David worked for 20 years with Firestone Tire and Rubber, 10 years with Goodwill Industries and currently was a Business Counselor for the State of Indiana. David's favorite activity was planning, producing and being the Disc Jockey of the Polka Party Radio Show which airs every Sunday morning. He was given the name "Polka David" since November 19, 2006. His love for Polka music provided the opportunity for him to play the Concertina that he taught himself to play with help from many friends and family. He was a member of several area Polka Bands, including The Jerry Kurdy's Orchestra. He enjoyed following NASCAR and the Miller Lite Team. David had built and raced his own race car at the South Bend Motor Speedway. For the last 26 years, he looked forward to and enjoyed going to the Indy 500 with his brothers. David was a fan of the Indianapolis Colts. He also enjoyed water skiing, jet skiing or just relaxing on the Pontoon Boat at the lake. He was a parishioner of Holy Family Catholic Church, a member of American Legion 357 and member and past president of Kiwanis Club. Visitation will be from 2:00 pm to 8:00 pm on Monday, November 28, 2011 in ST. JOSEPH FUNERAL HOME, 824 S. Mayflower Rd., where a Rosary will be prayed at 4:30 pm. A Mass of Christian Burial will be celebrated at 10:00 am on Tuesday, November 29, 2011 in Holy Family Catholic Church. Burial will follow in St. Joseph Cemetery.
